The morning your system will not start

Licence servers time out. A Windows update breaks the installation. The renewal was missed, or the dongle is in someone's other bag. Whatever the cause, the effect is the same: no Form F can be generated, and without Form F the woman in the waiting room cannot be scanned.

That is a strange amount of leverage to hand to a piece of software. This one has nothing to fail — no licence check, no activation, no server it needs to reach. Open the page and it works.

Most people who install it as a fallback end up using it every day. That is fine either way. It costs nothing and it does not care what else you run alongside it.

Twelve scans a month does not justify a practice suite

A consultant with a machine in her own room does a handful of obstetric scans between clinics. The obligation to produce a correct Form F for every one of them is identical to a busy imaging centre's. The software sold to meet it is priced for that centre, bundles a dozen modules she will never open, and expects a computer dedicated to it.

So the form gets filled by hand, on a pad, at speed, between patients — which is exactly the situation that produces a blank column.

This costs nothing, needs no dedicated machine, and takes about a dozen fields per patient once your centre details are saved. It is worth using for one scan a week.

What it actually does

It is a preparation tool. It checks that a form is complete and internally consistent. It does not judge whether a scan was clinically justified — that stays with you.

Asks for your centre details once

Centre name, address, PC&PNDT registration number, the doctor's name, qualifications and registration. Saved after the first form and greyed out on every one afterwards. What remains is about a dozen fields per patient.

Will not let an incomplete form be signed

A referring doctor with no registration number. Living children whose ages do not tally with the count. A declaration dated after the scan. An LMP giving a gestation past term. A running number already used. Each of those blocks the signature and says which field and why.

Keeps the M and Y numbers straight

The monthly and yearly running numbers are filled in for you and stay editable, so a centre adopting this mid-year types its current pair once and the sequence continues. Duplicates are refused. Signing locks the record; correcting a signed form voids that number and issues fresh ones, so the register never develops a gap.

Remembers the patient between visits

A woman returning for her next scan is one click. Her details carry across. The referring doctor deliberately does not, because she may have been sent by someone else this time. If enough months have passed for a new pregnancy, the LMP and living-children figures are cleared rather than assumed.

Prints on A4 at full size

Both pages, all four sections, every numbered item and sub-point as the form is prescribed, including Section C in full and the thumb-impression block. No scaling to fiddle with. The Marathi declaration uses a font served from this site, so it renders identically on every machine.

Gives you the register as a spreadsheet

Every signed form in yearly-number order, exported as CSV, for the register and the monthly return. The whole set backs up to a single file you keep.

What it does not do

It does not submit to any portal

Online entry, where your state requires it, remains a separate step done by you. Nothing here asks for portal credentials and nothing stores them.

It does not make you compliant

It removes the commonest defects in a record. Everything else about running a centre lawfully is unchanged, and responsibility for a signed form rests with the practitioner who signs it.

It does not sync between computers

Records live on the machine they were entered on. That suits one computer and one person keeping the register, however many scanners a centre runs.

Why is it free?
Form F is a legal obligation, and metering a legally required document would be a poor way to begin a relationship with a clinician. It also costs almost nothing to run, because there is no server holding your records. Tools added later may be paid; this one is not.
Can I use it alongside my existing software?
Yes, and plenty of people do. It does not integrate with anything and does not need to. Keep it open in a tab as the fallback for the day your main system will not start, or use it as the everyday form and leave the other software for what it is better at.
What if I close the browser or the computer restarts?
Your records are still there. They are saved on that machine as you type, not held in the page. What would lose them is clearing the browser's site data or moving to a different computer, so the tool prompts you to take a backup and restoring one is a single file.
Does it work without internet?
Once it has been opened on that machine, yes. It caches itself, so a dropped connection mid-clinic does not stop you printing. The first visit needs a connection.
I am picking up an existing register. Will the numbers clash?
No. The monthly and yearly numbers are editable. Type the pair you are up to on your first form and everything after that continues from there. If you enter a number already used, it refuses and tells you which record has it.
Does it work outside Maharashtra?
The layout follows the central prescribed Form F. States and district authorities add their own annexures and expectations, so print one and check it against what your Appropriate Authority accepts before adopting it. Tell us what differs and it can be accommodated.
